Welcome to SE Family Care Center, LLC.
We are part of the home and community health services in Minnesota. We work with individuals, families, and case managers, to provide the best personal care assistant (PCA) services to all clients of all ages in the comfort of their homes.
PCA program and services are free to individuals who qualify for Medical Assistance. You can apply through your local county. We can assist an individual with the paperwork needed to complete for Medical Assistance. Please visit our service section for more information.
PCA Services that we provide include homemaking, cleaning, meal making, eating, grooming, toileting, bathing, medicine reminders, shopping, transportation and getting around for appointments, assisting with mobility, bedding, and shopping.
We are accepting new clients. If you are currently with another home health care agency, we accept the transfer and would be more than happy to work with you.
We provide staffing for PCA Services to individuals who are in need of care, but also we are able to provide employment for a family member who is taking care of a loved one to get paid for providing care.
